New Patients

Welcome to PREG! We are so excited that you have chosen our clinic to provide your fertility care. Rest assured we will be with you every step of the way! Hereís some information to help you prepare for your first visit.

The initial consult with your physician is a comprehensive evaluation of both your history and medical records and your partnerís medical history if applicable. The appointment typically lasts 45 to 90 minutes. During this visit, you will have the opportunity to ask questions and discuss a plan for further evaluation and treatment. If you are attempting pregnancy with a partner, we strongly encourage that both partners attend this appointment.

Please complete the New Patient forms prior to your visit and bring your insurance cards and a photo ID with you to the appointment.

What to Expect at Your First Visit

To make the most of your first visit, please plan to arrive 10-15 minutes early for your appointment. If you have not completed your New Patient Forms, make sure to arrive a minimum of 30 minutes prior to your scheduled appointment time.

Upon arrival you will check in with a member of our staff at the reception desk. During this time, we will collect your insurance information, ID, and take your picture for your electronic medical record. If you do not have insurance coverage and are paying out-of-pocket, the cost of your new patient visit will be $200.

After check-in you will meet with your physician. When you finish with the doctor a nurse will review all the information and provide you with another opportunity to have all of your questions answered. As part of your initial consultation, patients may also speak with their financial counselor. In this meeting you will review the benefits our financial department has obtained from your insurance provider prior to your appointment. During this time, we will also review any financial questions you may have. If you prefer, a member of our financial team will call you to discuss insurance benefits.

Most patients will have some of the diagnostic lab work done at the initial consultation. We may not be able to get all of the lab work, depending on where you are in your menstrual cycle, but we will get as much as possible to start formulating a treatment plan based on your lab results.

Appointment Checklist

Being prepared for your appointment will help to ensure that your visit runs smoothly. Therefore, please prepare the following checklist items before you arrive for your consult. You will need to bring the following with you to your appointment:

  • Completed medical questionnaires (for both partners if applicable) on the New Patient Forms.
  • A copy of your medical records from your OB/GYN, primary care physician, or another fertility program.
  • If you have had fertility testing or treatments prior to your visit, please specifically note to have these records sent. Click here for a Records Release to request your medical records. Authorization for Release of Medical Records

  • Insurance cards for both partners
  • A referral from your primary care physician, insurance company or other third party benefit company, if applicable.
